• Passport in original that should contain 2 blank pages and should be issued within last 10 years and from North & East India and residing in North & East India for last 6 months along with the old passports if available and if old passports lost, attached the copy of FIR.
• 1 visa application form duly filled and signed (find attached)
• 2 recently clicked photographs with white background
• Typed Covering letter mentioning purpose of trip,travel dates along with the Passport details
• Air tickets
• Travel Insurance
• Last 3 months bank statement
• Last 3 months salary slips
• Last 3 years IT Returns or Form 16
• NOC from employer mentioning travel dates
• Proof of Self Employment in case of self employed.
• Hotel Reservation as per the Ticket enclosed
